It sounds as though you are well on your way.  Pretty soon the cat may simply regard Max as furniture.

Rules for the dog
1.	No pooping or peeing in the house 
2.	Let someone know if you have to poop or pee 
3.	Chewing is allowed only on designated items - your toys 
4.	No dogs on the furniture 
5.	Sitting on Poppa's lap does not count as sitting on the furniture 
6.	No humping 
7.	We do not have to sniff every single leaf or blade of grass 
8.	The big crazy man always wins
Rules for the human
1.	Always, always, always take at least one extra bag 
2.	 Walkies are required at 6:00AM,  mid-day,  after work or before going to bed regardless of how either of you feel about it 
3.	if  the dog has done his business,  you can turn around and come home 
4.	You are responsible for the consequences of a missed walk or any unfinished business.  
5.	The walk does not have to conform to your notion of geometry or urban planning - a walk is a walk 
6.	Keep the commands simple 
7.	Liver snaps are an acceptable bribe or payoff 
8.	It is OK to wait

Print and post on the refrigerator about 14" above the floor -- if it is too high,  how will he be able to read it?

I don't have rules for the cat. You can tell a cat....but not much.
